CARE MANAGER/ ASSESSOR- MCKEAN COUNTY

 The Care Manager/Assessor conducts home visits, completes assessments, develops care plans, arranges services, monitors ongoing needs of older adults, provides information on community services and supports, maintains a working relationship with service providers, ensures services are being provided. The Care Manager/Assessor will also perform Protective Services work taking reports of need for older adults in need of protective services.

Key Tasks and Responsibilities
· Complete comprehensive medical assessments.
· Develop and manage consumer’s care plans.
· Identify any existing community supports.
· Coordinate care with family, community and professional support.
· Document different types of financial information in conjunction with the consumer’s care plan.
· Assist consumers with medical assistance benefits.
· Monitor and evaluate the quality of consumer’s care.
· Make difficult decisions regarding consumer’s care.
· Provide consumers with risk assessment and risk management plans.
· Encourage, assure and support consumer’s Independence.
· Provide on-call and investigative duties for the Protective Service Program.
· Additional tasks may be assigned as needed.

Applicant must:
· Have strong time management and documentation skills and cope well under stressful situations
· Be strong consumer advocate who can maintain a balance of objectivity and empathy
· Have excellent communication skills and quick decision- making abilities
· Enjoy working in a team environment providing support to others
· Have advanced computer and documentation skills
· Have a reliable vehicle for work related travel, auto insurance and a valid driver license
· Pass a criminal background check, child abuse clearance and possible FBI clearance
· Take pre-employment drug and alcohol test
· Must be available to cover on-call hours when required (approx. 6x per year- paid at a premium rate)

I. Education and Experience Requirements
· Bachelor’s degree (BS,BA) in human services or related field and one year of related experience or Associate’s degree in human services or related field, LPN or RN license and at least two years of related experience
· Computer proficiency
· Valid Driver’s License
· Reasoning and Decision- Making Abilities

· Travel in Various Weather Conditions
· Work Related Travel

The normal workday hours are Monday through Friday 8:00am to 4:00pm but Care Managers are required to provide Protective Services which will require Care Managers to work after hours on a rotation basis.
